The claim form is called AA1. It is free: DWP posts one when you ring to claim, or you can download and print it.
Getting the form
Ring 0800 731 0122 and ask for the AA1 claim form, and DWP posts one to you. Or download and print it from GOV.UK – see our guide to the AA1 form. Either way it is free.
Ringing first is worth it: The claim can start from the date of the call, if the form is returned within 6 weeks.

Sending it back
Post it to "Freepost DWP Attendance Allowance" within 6 weeks of your call. No stamp or postcode is needed, and nothing else goes on the envelope.

Can I get the AA1 form online to print it?
Yes. You can download and print the AA1 from GOV.UK, or ring 0800 731 0122 and DWP posts one to you. If you ring first, the call date can count as the claim date.
Is there a different form for someone in Scotland?
Yes. In Scotland, Attendance Allowance has been replaced by Pension Age Disability Payment. It's a different form and a different office.
Does it cost anything to get the form?
You can apply to DWP directly, and Citizens Advice can help at no cost.
