Short answer
The claim form is called AA1. It is free, and DWP posts you one when you ring to claim.
Getting the form
Ring 0800 731 0122 and ask for the AA1 claim form. It is free from DWP - you do not need to find or print it yourself.
Sending it back
Post it to "Freepost DWP Attendance Allowance" - no stamp is needed - within 6 weeks of your call.
